The majority of Scheme members receive an annual benefit statement which sets out the anticipated benefits available at particular dates including earliest retirement date and would therefore not generally require a further estimate of benefits. From time to time however, a Scheme member may have a requirement to obtain additional information and to this end a form is available for completion as shown below. The Scheme member should read the notes identified on the last page of the form.
